In the fast-paced world of clinical informatics, Alcidion, a leader in smart health solutions, is dedicated to utilizing technology to ease the cognitive load on clinicians. Their flagship platform, Miya Precision, plays a crucial role in managing patient flow and ensuring accurate patient placements in hospitals. Recently, Alcidion undertook a significant architectural overhaul by migrating from a legacy Microsoft SQL Server to Google Cloud’s AlloyDB for PostgreSQL.
Challenges Faced
Alcidion encountered several technical and operational challenges with their previous SQL Server setup:
- Operational Overhead: Managing SQL Server backends involved considerable manual effort, including load balancing and space management.
- Performance Latency: Processing complex JSON data took up to 30 minutes for certain tasks, hindering efficiency.
- Stability Issues: The team required a more stable Kubernetes environment that could scale without constant administrative input.
Migration to AlloyDB
To address these challenges, Alcidion utilized the Database Migration Service (DMS) to transition to AlloyDB. The migration was notably efficient, taking less than a month, with the core database move completed in just one and a half weeks.
Custom synchronization tools and Google Cloud’s managed services enabled a final transition window of only 15 minutes. This was accomplished by setting up a new Google Cloud instance that was synchronized with the existing one, allowing a seamless cutover.
Results Achieved
The migration to AlloyDB has yielded measurable benefits for Alcidion and its healthcare clients:
- Accelerated Data Processing: Tasks that previously took 30 minutes now complete in just 5 to 60 seconds, thanks to the new processing capabilities.
- Increased Stability: The new environment has significantly reduced disruptions, moving away from the frequent issues experienced with SQL Server.
- Lowered Cognitive Load: Simplifying the backend has allowed Alcidion’s team to focus on innovation, enhancing tools like predictive analytics and generative AI for clinicians.

Alcidion plans to build on this modernization by exploring further enhancements:
- Columnar Engine: The team is investigating the columnar engine for advanced query optimization and real-time analytics.
